Pocket Cards

It is always helpful to have reference materials readily available at the bedside. We have designed a CAM-ICU pocket card. These cards include detailed instructions for each of the four features of the CAM-ICU in addition to a complete guide to the RASS scale. Below is a link to the Pocket Card. This file contains two pages (The 1st page is the outside of the card and 2nd is the page is the inside of the card). Please contact us for a Microsoft Word file to alter this card to fit your institution’s needs (i.e., replace the RASS with the sedation scale that your institution uses).

Directions for printing the pocket cards:
1. Print the attached file
2. Copy this file from two single sided pages to 1 double sided page (front and back)
3. Cut the page in half (cut on dotted line).
4. Fold each half in half vertically (like a book) One copy makes 2 cards.

*We recommend using card stock and having the cards laminated.
